Health Homes is a new comprehensive care management resource that will be available to offer assistance to eligible students at Medaille, thanks to Say Yes Buffalo now piloting those services to students on college campuses. This resource is available to all eligible Medaille students up to 21 years of age, not just Say Yes students. View the Health Home Program brochure.
A “Health Home” is not a physical place; it is a group of health care and service providers working together to make sure eligible individuals get the care and services they need to stay healthy. Once a student is enrolled in a Health Home, the Family Support Specialist / Health Care Manager from Say Yes Buffalo will work directly with the student to develop a care plan and help meet their needs. A care plan maps out the services they need, to put them on the road to better health. Some of the services may include:
- Connecting to health care providers
- Connecting to mental health and substance abuse providers
- Connecting to needed medications
- Help with housing
- Social services (such as food, benefits and transportation)
- Other community programs offering support and assistance
Faculty and staff members can help by referring students who they feel could benefit from Health Home services. To refer a student, please fill out the Wellness Center Case Management Referral Form. The Wellness Center team will then be able to help the student navigate to the Health Homes program, or if the student does not qualify for the Health Homes program for any reason, the Wellness Center Student Advocate will work directly with the student to help meet their needs.
